A sewage backup is never a mop-and-fan job. The water is contaminated, the materials it touches are contaminated, and mold is the second problem, not the first. EPA’s instruction is explicit: “If the water and/or mold damage was caused by sewage or other contaminated water, then call in a professional who has experience cleaning and fixing buildings damaged by contaminated water.” Do not shop-vac sewage. Get people out of the wet area, stop the backup, hire restoration that handles Category 3 water, and only then deal with mold on anything that stayed wet. Cost is higher than a clean-water leak. Insurance often requires a water-backup endorsement, and flood is a separate policy. None of that coverage is guaranteed.

Clean water from a supply line is a drying problem if you catch it fast. Sewage is a contamination problem on hour one, and a mold problem if the structure stays wet.
IICRC S500, the Standard for Professional Water Damage Restoration, classifies this as Category 3 water. IICRC’s public position statement describes Category 3 as “grossly contaminated” water that “can contain pathogenic, toxigenic, or other harmful agents.” Examples include “sewage” and “wasteline backflows that originate from beyond any trap regardless of visible content or color.” Color is not the test. A backup that looks like water and came from beyond the trap is still Category 3.
EPA’s commercial remediation guide: if you know or suspect the water was “contaminated with sewage, or chemical or biological pollutants,” OSHA requires PPE and containment, and “An experienced professional should be consulted if you and/or your remediators do not have expertise in remediating contaminated water situations.”
CDC on flooded homes: floodwater “may have carried sewage or chemicals into your home,” which can expose you to “viruses, bacteria, disease carriers (such as mosquitos), and parasites, as well as mold.” Mold is on that list. It is not the only hazard, and it is not bathroom mildew.

The 10 square foot guideline on DIY or professional assumes ordinary indoor mold on a hard surface, not sewage. Do not use it on a basement backup.
A household wet/dry vacuum turns sewage into airborne droplets and permanently contaminates the vacuum. You do not want that tank, filter, or exhaust in the house.
Do not mop sewage through living space. Do not run the HVAC to “dry the basement.” EPA: “Do not run the HVAC system if you know or suspect that it is contaminated with mold.” The same logic applies when the water itself is sewage. Do not pull carpet and stack it in a bedroom to “save it.” CDC: “Remove and discard drywall and insulation that has been contaminated with sewage or floodwater.” Do not mix bleach and ammonia. Get people, especially children and anyone with asthma or a weakened immune system, out of the affected area. CDC: people with asthma, COPD, or weakened immunity “should not stay in a moldy home, or even be there while it is being cleaned.”

EPA: “Porous materials that are wet and have mold growing on them may have to be discarded.” After sewage, many of those materials have to leave whether or not you can already see mold, because you cannot clean the core.

CDC’s disaster cleanup list is blunt: throw away mattresses, carpeting, stuffed animals, pillows, and similar items you cannot clean or dry quickly.
If mold is already visible on what remains, you are in S520 remediation on top of the Category 3 restoration. Dead growth is still a problem. EPA: “Whether dead or alive, mold is allergenic.”
Call contaminated-water restoration immediately. Call a mold remediator when growth is visible, when porous materials sat wet past about 48 hours, or when an independent assessor has already named square footage.
Call now for a toilet overflow onto flooring or into a wall, a floor drain or basement cleanout that pushed sewage up, or floodwater that may have mixed with sewage. CDC: if a flooded home has been closed up for several days, “assume your home has mold.” If you already see growth on drywall or smell a musty cavity, do not open those walls yourself.
Do not hire the restoration company to “inspect for mold” and then award itself the remediation. Get the water stopped and extracted, then a separate assessor if mold is in play. The role split is on who to call.
Clean-water restoration is equipment and time. Category 3 work adds PPE, extra disposal, disinfection, and more demolition because porous materials cannot be negotiated back into service. If mold follows, you stack an S520 job on top.

Use the cost guide and the mold calculator for the mold line, not as a quote for the sewage cleanup itself. Ask every bid whether Category 3 procedures, disposal, disinfection, rebuild, and clearance are included.
Do not assume the homeowners policy will pay. Standard policies often cover sudden accidental discharge from plumbing inside the house. They commonly exclude water or sewage that backs up through sewers, drains, or a sump. That gap is why carriers sell a water backup or sewer backup endorsement, often with its own limit, commonly in a range such as $5,000 to $25,000 depending on what you bought.
Flood is a different exclusion. Water that rises from outside is a flood claim, usually under a separate NFIP or private flood policy. FEMA notes that NFIP flood policies “will not cover damage from mold” in ordinary circumstances. Outdoor sewage-laden floodwater is the flood mold path.
Even when water damage is covered, mold cleanup is often capped. The Insurance Information Institute is plain that policies will not cover damage due to lack of maintenance, mold, or infestation as a general rule, while accidental discharge from plumbing can be a covered peril. The sewage jobs I see go wrong in the first hour. Someone pulls out the shop vac, someone else turns on the air handler, and a third person starts cutting wet drywall with no gloves and no plastic.
You do not need to identify a species. Treat the water as contaminated, get people out of it, and hire a crew that does Category 3 work every week. If the board stayed wet, remediate it as mold after the sewage is gone. Those are two standards, S500 then S520, not one spray.
Read the policy before you take the first restoration estimate as “insurance will handle it.” Backup endorsements and flood policies are easy to confuse in a wet basement.
